Waiver Terms and Conditions

I waive and release Another Level Basketball Camps from any and all liability from injury and illness going to camp, while at camp, or returning from camp. I, as parent/guardian, have actual knowledge and appreciation of the particulars of the camp and hereby voluntarily consent to said minor's participation, and assume the risk arising therefrom. I hereby give my permission for emergency medical treatment in the event I cannot be reached.

Another Level Basketball Camps requires a physical examination within the 12 month period of the date the camper is scheduled to attend camp. I confirm that my child will meet this requirement and I will notify Another Level Basketball Camps of any condition that may prevent him/her from participating in any/all camp activities.

NOTICE:
Per the Arkansas Activities Association, an Athletic Release Form must be completed (On Site) prior to a student attending or participating in our camps, that is organized, led, or coached by any person affiliated with a AAA member school other than the school in which the student is currently enrolled, whether you are Coach, Registered Volunteer, or anyone who assists in any capacity with the coaching or training of the school team completes this form prior to working with any student who is not enrolled in your school or feeder school.

Also, ALBC reserve the right to refuse instruction and/or accept campers foul behavior/conduct which is not conducive to ALBC standards of instruction.
